Change of address

Please use this form to inform us of any mailing address change.

 

Change of Beneficiary / Change of name / request for duplicate certificate

Please use this form to change the named beneficiary or beneficiaries on your Certificate, change how your name appears on your Certificate, or request a Duplicate of your previously issued Certificate that has been misplaced.

If you are requesting a Change of Name that reflects a previously made legal change of name (eg. marriage), please submit a photocopy of any supporting documentation with this form.

Please note that we require this form to be notarized before being returned to us.

 

Transfer of Ownership

Please use this form to transfer the Ownership of your previously issued Certificate to another party. Before completing and returning this form to us, please carefully read this form to understand its implications.
